Resmed – Your next RePAP opportunity may be easier than you think.

Your existing patient population likely includes people who are ready for CPAP device replacement (RePAP). The challenge is identifying those patients and knowing when to reach out.

Patient identification is the first step toward building a more proactive RePAP program, and other HMEs have found ways to make that first step more manageable.

See what may be within your existing patient population

When Performance Home Medical1 set out to bring more structure to its RePAP initiative, the team used existing reporting to build a targeted patient list.

The organization identified patients who had been on a PAP device for at least five years and had ordered supplies within the previous 12 months. Using those criteria, Performance Home Medical reported identifying nearly 10,000 eligible patients, with close to 500 additional patients each month entering that category.

The takeaway: Understanding the patient information already available to your organization can help you identify and prioritize patients who may benefit from replacement equipment.

Move beyond manual patient identification

Nunn’s Home Medical Equipment2 had long encouraged eligible patients to replace their PAP devices, but its process was largely manual and inconsistent. Tasks were scheduled years in advance, outreach happened sporadically, and tracking was difficult.

The team wanted a more efficient way to identify eligible patients, communicate with them and manage follow-up. By leveraging existing reporting to pull patient data and digital tools to support outreach, Nunn’s created a more streamlined approach to reaching eligible patients.

The takeaway: Available tools and patient information can help HMEs move from manual patient identification toward a more streamlined approach to finding patients who may benefit from replacement equipment.
